If you look up food items and there’s all different portion sizes for the same calories how do you know which one is right?

It depends on the food.
For packaged food, I always look for an entry that corresponds to the info on the package. For whole foods, the USDA food database is a good source (legacy entries).
For prepared foods, it's best to use the recipe builder or meal function with the individual ingredients. Or, if you didn't cook it yourself, I look for supermarket versions of the same meals, or well known restaurant chains that publish their nutritional data.1 -
It takes guesswork. I tend to log what I consider the "worst case scenario" and that way if it has a lot of calories I'm accounting for it.0
