    Can you ask for a referral from the surgeon to a dietician who can advise you how fast you can safely lose weight before surgery and the best way to achieve that?

    When is the surgery.

    I’m assuming you’re carrying a lot of fat hence the doctor’s wanting 20 lbs off. If you take in 1,000 calories a day less it’ll take about 10-12 weeks. Longer if you don’t cut that many calories daily and shorter if you cut even more daily calories however shorter is not advised unless your doc wants the weight off quicker.

    Why are you wanting to loose weight before surgery? When is the surgery? Check with your doctor/surgeon about whether loosing weight before that is really a good idea. Yes, being less overweight generally improves surgery outcomes. But a crash diet or sudden weight loss will not bring your risks down to the same as a person who is lighter. Many of the health risks of overweight weight (i.e. paque in veins, insulin resistance/diabetes, high cholesterol, high blood pressure.) take weeks or months to reverse. Dropping weight a couple weeks before surgery won't necessarily make these changes in time for the surgery. You also need to make sure that your eating is healthy and supplies plenty of protein and vitamins, which will be important for healing.
