According to the app, it says cream of wheat cereal has 14 g of protein for serving and on the box it says 4 g. That’s a huge difference. I am a dialysis patient and I am trying to keep my protein numbers up.

Go by the box, always go by the box. Not only do manufacturers change recipes and formulas from time to time, entries in this database are international, so the information may be different depending on location. Beyond those possibilities, this is a crowd-sourced database that is only minimally curated by MFP staff. With numbers like 14 and 4 involved, it's pretty likely that whoever created that entry made a typo that is now memorialized forever.3
