I've been reading lots of mention of this and have been curious. What is it? What does it involve? How can I become more conscious?

I think it entails different things to different people.
For me, it means no processed foods, no sugars (except from fruits), I don't drink anything except water, lots of veggies, fruits and lean meats.0 -

Haha, that was my birthday last year. And I most certainly devoured it.0 -
Others have already stated that this phrase is ambiguous at best.
Generally accepted practices that fall under the heading include the following but are not limited to:
Actually purchasing unprepared foods and cooking them to prepare a meal. (prepackaged quick fix meals loaded with sodium and preservatives do not qualify)
Moving away from stuff loaded with Hydrogenated or partially-hydrogenated anything, moving away from fructose corn syrup in most any form or name.
Including a variety of colorful fruits and vegetables in the diet.
Moving away from bleached flour and cutting back the starchy carbs in the diet to varying degrees. This is a fairly obvious consequence of the previous point when budgeting calories. When you increase something, something else gives way.
