Accurate HRM alternative?

Hi folks!

My mom and I work out together often, and she is more interested in fitness recently. I got a HRM a few months ago and LOVE it. She's been looking at it enviously, and I was going to get her one for Christmas, but she asked her doctor and he said it wouldn't work for her. She had a problem years ago with palpitations and is on medication to keep her heart rate slow. Would this wreck her accuracy? Or does it go based off of some sort of ratio increase (if her resting heart rate is slower than most, would the increase while working out make it accurate)?
Does anyone know any other gadgets that give accurate calorie counts *without* using just the heart rate?

    It shouldnt affect the accuracy of each individual beat just the average.

    So would a regular HRM work? Her pulse only gets to 120 - 140 max when exercising.
