Template to track weight with LibreOffice—help!?

Hey all!
I'm looking for a template to track my weight on LibreOffice and was wondering if someone has something on that? I found this one, which would be perfect but sadly doesn't work on the newer versions of LibreOffice—and I'm not tech-savy enough to even attempt to fix it.

It crashes opening in MS Excel 2010 so I can't resave it but I'll look at home tonight on Libre Office.0
