Im back again...here are my goals!

Hello! My names Mir, I've been a member with MFP since before 2012. Been dieting/working out on and off for the past 2 years. It's been about a year since I started a serious exercise/eating program but am determined to start for good this time and stick to it! Feel free to add me if you're in the same boat as I or whoever is looking for additional support and motivation!

5'3" (Haven't weighed myself yet but I could assume im a good 150 right now.)
Long Term Goal in 6 months: Lose at least 25 lbs.

My goals for December:
Start and maintain a consistent workout routine.
(3x a week C25K --
1-2x week alternative cardio --
1x a week yogaaaa --
incorporating body resistance muscle training throughout the week, currently doing 30 day ab and booty challenges from Pinterest)

Monitor and reduce unhealthy eating habits, trading said bad habits for new healthier ones.
Increasing vitamin intake.
