I need help creating an exercise plan

I work best if I have a routine say every Monday I go to the gym however as of lately I don't feel like my exercise has been paying off and so I want to mix it up a bit. Normally when I go to the gym I spend an hour on the cross trainer at level 80 or above and on elevation 5 I don't feel a sweat or that tired when finished but I am by all means not fit.
At my gym there are cross trainers, bikes, rowing machines and treadmills however I can barely run 5 minutes so that probably isn't the best to use.
Also I would like to incorporate weights and so plan on using online videos such as the 30 day shred and others in this range. Can someone help me to plan what to do each day so that I am not just focusing on one area of my body (I feel like my arms are neglected :L ) Also this new plan should push my body into burning fat and so actually allowing me to make progress with this weight loss.

edit** also I have netball practise for 2 hours every wednesday.
