Food scale portions and which entries are the right ones?

I've been successfully losing weight just estimating or volume-measuring my portions. I know it's not accurate and I have no doubt I'm eating more than I think, but as I said, weight loss has been successful, so I've been okay with it.
However, I'm getting closer to goal weight and need a smaller deficit, so I'm thinking about getting a scale.
Question: how do you know whether a portion size is supposed to be weighed raw or cooked? Is 3oz of chicken before cooking or after? Etc.
Also, I've seen several references to the "official" entries in the data base versus the ones others enter than may be wrong. How do I tell (from my phone) which is which?

    Type in chicken raw or cooked then select serving size and it will be a drop down menu. Select grams or ounces.

    also, try to pick the items that DON'T have an asterisk (*) in front of them. Asterisk means someone entered it from home, & may not be accurate. It may be - I try to use package info when I enter stuff - but you're safer with the non asterisked items.

    This person is using the phone app which does not show an asterisk.
