Creating a good home workout?

I currently don't have a decent workout plan, as I've said in a previous post I have a habit of just doing what I feel like that day which kind of leaves me not knowing what to do and sometimes I just don't workout.
At home we have a treadmill (fixed incline), resistance bands, 5lb ankle weights, dumbbells and a barbell.
I can't lift too heavy for medical reasons but other than that I should be fine.
Any help?

Sorry I was in a rush to get this up and completely forgot to add that bit
Lose fat while still retaining muscle and increasing strength if that is entirely possible
