PCOS with severe knee problems

Hello!

I was diagnosed with PCOS about 3 years ago. I have been attempting to loose weight for about 8 years with little to no long lasting effects. I have very serious knee problems in which they dislocate, with (sometimes) the simplest of movements. I have had multiple surgeries to correct one of my knees, knee injections into both knees, physical therapy (so much therapy! )
So working out can be difficult, painful and unsuccessful. I have tried many types of diets: low-carb, no-carb, diet pills, water pills, veggie diets, green shakes, shake ons and so many more.

For those of you with PCOS, what kind of diet plans work best for you? How do you work with the insulin problems and food?

For those of you with knee problems (or with both) how do you get the necessary cardio? What workouts are really, really low impact, but really effective?
