Metatarsal Inserts for athletic shoes

Just thought I'd toss this out there to see if anyone may have any leads for something like this. For the past four years I have used Lynco L405 Metatarsal insoles whenever I do something very active (walking, occasional run, treadmill, etc). I really liked these and I've pushed my last pair beyond the ideal life of them.
The guy I used to get them from (and who would put an additional cushion on the front for me) went to work elsewhere and I haven't had success finding anyone else who can customize the inserts for me. I know that Lynco (made by Aetrex) has put a newer version out - but I tried those and they are nothing like the former version.
Anyone have any ideas on the topic? Normal shoe cushioning is not enough and I need the metatarsal support (I have been attempting to make-do with the type you 'stick' in and they su*k). At this point I'm just trying to cast a wider web on any potential replacements.
I've tried Spenco, but they are like wearing nothing at all. I tried a couple of off-brands, but I am just hoping that I can be lucky enough to find something comparable to the old Lynco L405.
