So let's say I need like 4 oz of chicken. I should measure 4 oz raw? When I cook it it becomes like 3 oz or something ? Is it still the same !?

Yes, weigh it raw and use an entry for raw chicken.

If you are cooking multiple portions, weigh it raw for X number of portions, then once cooked, weigh again and divide by X for the amount of cooked food per portion.
