Getting back on track

Is there anyone that has lost the weight and then gained it back, for one reason or another, and then can't lose it again? In 2012, I had a heart attack at 40 years old, due to small arteries and anemia, I was told. I did awesome and lost 50 pounds. I then had a hysterectomy and gained it all back in the last year and a half. I'm just so super tired all the time so that I can't get motivated. I'm so tired that I start falling asleep at my desk at work. I'm working out two-three days per week now but I'm gaining and I've been doing it for three months. I realize this happens sometimes when someone starts to workout but my jeans are getting too tight to put on. I know my eating is not the best also, so I'm trying to work on that. I've stopped drinking soda, except sometimes on the weekends. Any suggestions from someone who lost the weight, gained it back and is now trying to lose it again? Or, I guess just suggestions from anyone who may have some pointers.

    Track your food and use a food scale. If you're not looking at what you're eating, you don't know if you're in a deficit.

    As someone who thought I'd messed up my metabolism by yo-yo dieting and swore it was harder to lose weight now that I'm older, once I started tracking calories in/calories out and weighing my portions, it's been like clockwork.
