Need help getting back on the wagon - how did you do it?

I've fallen off the wagon. I haven't been to the gym in 2 weeks, I'm up 3 lbs, and I am blowing past my calorie target on a daily basis.
I need a little help getting back on.
I'm trying to set myself up well for the week. I've already meal planned and cooked for the week. I've got it all portioned and packed. I asked my Dad to help and give me a backup wake up call in the am so I can get to the gym before work.
Basically I'm doing everything I can think of to set myself up for the week.
I'm sure I'm not the only one who has ever fallen off. I'd really appreciate knowing what you did to get back on. How did you get re-motivated?

I've been at this in one way or another for going on 3 years now. Each year I looked back at the year before, realized all of the mistakes that I made that I didn't realize I was making, and said "crap! Imagine how much further along you would be if you knew then what you know now."
This year I said to myself: 2013 you were figuring it all out. 2014 was your warm up. 2015, you crush ****! I've had my first bad week in a long time - personal reasons, but I've been at it for so long that I actually started to get depressed as I felt that drop of poison slip in..."doesn't it feel better to sleep in and not worry about it? Just relax. You don't need to do all of this. Your wife will love you no matter how you look." When I realized I was allowing myself to regress, I asked myself "January 1, 2016, what are you going to say about yourself looking back over 2015? Don't let it be "damn! I should have done better."0 -
Hi Jessy....are you depriving yourself with certain food groups? Do you have treats? If not maybe incorporate them into your calories .... Just get up and do it ...YOU can do it ...YOU will do it ...and you will be more than happy you did .....You know my story so if I can do it so can you ....If you need help with food take a look at my diary ....if you need help in general just let me know .0

Thanks guys! I appreciate the responses.
@RossAH - That's a great question to keep focused on. I've been trying for years. I will be pissed if I get to 2016 and I feel like I really could have done better. I was kicking myself on NYE 2015 for the same reason.
@ms_smartypants - No. I didn't have any treats built in. I think that was killing me. I also think I'm putting carb cycling on the back burner for now. I need to simplify and just get some momentum going. I'll take a look at your diary too! Thanks!
For this week I'll simplify my goals ... I'll complicate it later when I'm hitting these every week.
