Underestimating food & Overestimating Burn?!

I've heard so many people chalking up stalled/failed/otherwise-not-working weight loss to either their underestimation of how much they eat or overestimating how much they burn via exercise.

So is the best way to carefully measure your food so you're more aware of just how much you're eating, then exercise with regularity? Then from there you can either adjust your intensity or frequency/duration of your exercise if you're not seeing any results?
