Are the manufacturer's nutrition facts labels for cooked or uncooked weight?

I assume that if the food is in an uncooked state, then the nutritional values on the label are for uncooked portion sizes. I also assume that it is for the edible portion only. But I don't want to assume anything.
I know that these are silly questions, but I am not sure that I am measuring my amounts correctly and may be understating my calories.

I always assume the calories and nutritional values are for the product as it is unless it states when cooked or something similar.
Eta I do weigh out the serving size according to the gram weight per serving.0
