Hello.....3rd time a charm?

I started MFP back in 2010 and managed to lose 45 lbs. I kept it off for a few months but quickly went back to old horrible habits. I continued to log though my weight crept on and fast.
I stopped logging for awhile but came back and managed to lose 50 lbs (40 of which I had lost before and gained back). I felt amazing but this time I lost all of it with the help of medication and the minute I stopped taking it I gained back 15 lbs. I managed to keep the rest off for a year before it all came rushing back on very quickly.
The sad thing is, I know what to do and how to do it but I ignore the fact that I should be doing it so I don't. In other words, I take the lazy way out just to eat something that won't bring me happiness and get in some TV time after a long day at work.
My job is way less stressful now but I also sit for 8-10 hrs a day. Because my job as a pharmacy tech requires me to stick to a deadline of getting meds pushed out it is not easy to get up and move when I need to, much easier to do before when I was a medical assistant in a very busy family medicine practice.
As of right now, I will be 35 in 8 days. I want the last 5 years of my 30s to be my best years (and getting better going into my 40s).
I have very specific goals in mind and they require determination and a solid plan. This time I will get there.
I would love to have people in various stages in their journey, as long as you're motivated and willing to help keep me accountable. I would really like it also if a few people were close to me as I need people who will force me to the gym every day (I am in St. Louis).
