Getting back on track

So from April 2015 - January 2016 I lost about 43 lbs. I was only 13 lbs away from my goal weight and was starting to feel and look really great. Then for no real reason I just totally lost all my motivation. I stopped working out and counting calories. I completely stopped logging into this app. Every week I would say "next week" and then next week would come and go. Now here we are at the beginning of summer and instead of losing that last 13 lbs I have gained back 10. So as of today I really am starting. I don't want to be a yo-yo. Back to working out, counting everything and logging into mfp religiously. Any motivation? Inspiration? Ideas to help get back on track?
