Preventing injury while lifting?

Seems pretty obvious, but after waking up to a swollen arm today, it made me wonder. I was working with my trainer, doing nothing really out of the ordinary except pull-ups. Those apparently kicked my butt and I didn't even know it at the time. My trainer was assisting me so I wasn't struggling too bad.
I never would have thought I would have a swollen bicep and forearm today. Been icing it all day and hoping that it gets better on it's own and I didn't do any damage. How do you push yourself without hurting yourself? Do you do anything besides stretch, warm-up and cool down? This just threw me off today.

No, it's swollen. It's limiting my elbow movement because of the swelling.0 -
Then I have no idea why it would be swollen. Generally speaking, there shouldn't be a reason for it.0
