Last week I didn't make one workout as I was sick - although I walked about 4 miles a day just to do something vaguely active.
I went back today and felt sick, I'd lost strength and even thought I might pass out at one stage. Can a week really make that much difference to my fitness??

You're probably still recovering, start slow. I had to take 10 days off from the flu that turned into bronchitis and the first week back things were as you described, after that it got back to normal fairly quickly.1

Probably still a bit sick. I did the same after flu. Thought I was well enough to exercise but actually felt terrible. I think I gave myself nearly another week if not an entire week and came back feeling awesome on my first full workout (with a little condition loss but nothing dramatic). So give it more time.1

You do not loose fitness in one week of "rest". You are probably not fully recovered and even though you thought 4 miles a day was not much, most likely it was. Being sick demands full rest so that all the calories (energy) you provide your body daily goes heal and recover you from that sickness.
Slowly get back to walking and then add some more each day. Your medicine is more time!2
