Keep losing motivation

I've been struggling for about 5 years with staying motivated for long periods of time. When I start working out and eating healthy I stay on track for atleast 2-3 months and almost reach my weight loss and strength goals but then I end up missing the odd workout and before I know it I completely stop eating healthy and am right back to where I started. I see the progress in the mirror when I get close and start getting excited I'm almost there so i don't understand how I'm always losing motivation and finding it so hard to stick with it.
Does anyone have any tips on how to keep myself going?

    If I'm not moving forward, then I'm heading back to where I started. I don't want to go there, so I keep moving forward, No matter how small the steps. You clearly have the skills and habits to make changes. Do you really want it? Only you can decide. You can do the right things even if you don't "feel motivated" on any given day.
    PS You are very fortunate to be able to get close to your goal in 2 months. I've been at it 2 years with steady progress and only now is my goal in sight!

    What is your motivation? Health? Appearance? Fitting into old clothes? My doctors had nagged me about losing, but for me the real goal was appearance and fitting in my clothes. So there's a pair of jeans laying on my night stand...every day I wake up and look at them, sometimes I try them on...but they don't fit so there's more work to be done. I also have a picture of myself at the beginning of my weight loss decision...if I start losing motivation, I look at that and remember that I may not be where I want to be yet, but I definitely don't want to go back to where I was.

    Everyone hits a weight loss plateau at one point or another and it can get discouraging if you don't recognize it for what it is and learn now to get past it.
