I normally only last a week with diet and exercise. Any advice on how to make it a lifestyle?

First off, don't think of it as a diet. Set certain macros (a calorie-intake that suits you, lower carbs, lower sugar and higher protein) and instead of eliminating food, just eat a smaller amount and make them fit into your set portions. After a little while, you will get the hang of it. Over time, your cravings and hunger levels will adjust. Stay persistant. As for the gym, start off with cardio until getting there is a routine. Then incorporate lifting. Make yourself stick with it for a month, you will see results in that time and the results/progress alone will make you have the motivation to continue.
