How to gauge an HIIT workout

What is the yardstick to measure an HIIT session as opposed to a LIIT session? How do I know whether the session I just had was "high intensity" or "medium intensity" or even "low intensity"? Is it based on your heart rate?
Also, is "intensity" only about sprinting, or could you also do a "high intensity" session by increasing the % incline on a treadmill? I'm new to this and I want to incorporate some HIIT into my routine but I have bad knees which makes running a challenge. How should I proceed? I've plateaued in terms of weight loss/ inch loss so want to change things a little bit without damaging my knees.
