I can't seem to change my daily lifestyle to what it needs to be. I fall off the wagon every other month.

It sounds like your wagon is too tall.

Don't jump into this with both feet. Change one small thing at a time and get used to that before you change another small thing.

To lose weight, I started with my original way of eating and either ate smaller portions of those foods or made substitutions with less calorie-dense foods that I also liked. My new way of eating is essentially my old way of eating with a few tweaks. It's easier to stay on the wagon because it's so close to my old way.

Even if you have goals other than weight loss, slow change will likely create new habits more easily than fast change.
