Weighing food and recording nutrients

Hello,
I am looking for help in clearing up a few questions I have.

Are the grams/ounces listed usually cooked or not cooked?
Do I weigh for instance; a piece of beef raw or cooked?
Do labels assume this?
Does a label assume I wont drain off any excess fat rendered during the cooking process or trim off excess fat prior to cooking?
If the label does NOT assume Ill drain off extra rendered fat, is there a way to recalculate the numbers?

    Weigh raw, as far as I am aware labels assume this.

    Not sure about the fat draining off bit though, I usually just enter the calories on the packaging. I do trim fat off and grill everything so that it drains away but have never recalculated the calories for that.
