Restaurant nutrition guides

Headed to a chain restaurant here. I checked the nutrition guide and prelogged entries. It states that sides are obviously not included in nutrition but I'm confused if the bun and onions and mushrooms are. It's a steak sandwich on an open face ciabatta with mushrooms, red peppers and onions. Says it is 473 calories. Do i keep the 473 calories and add calories for the bun?
It's 140 calories to add the steak to a salad but 450 for a 7 oz steak as an entree.

It should include everything that is part of the entree, so definitely the bun. Then you would add if you put ketchup on, since that's at the table. And you definitely aren't getting 7 ounces of steak on a salad...2-2.5 ounces sounds about right.1
