Measuring cooked?

Just want to double check on when measuring meat for example, are the listed entry amounts for cooked or raw?

Some are cooked, some are raw. Some are just wrong
Use database entries that clearly state whether they are for raw or cooked food, and if cooked, they should state how it was cooked. Then compare it to entries on the USDA nutrient database.
