Let's give this another try

I have on several occasions tried to get serious about my weight loss and overall health. I think I've finally reached a precipice! I just turned 36 a couple days ago, and started taking serious inventory of my life and goals.
I am 400lbs BMI 50+, hypertensive and pre-diabetic. I have pitting edema in my legs and Over the last year I've developed moderate back problems and worsening depression. I work a desk job and live a largely sedentary lifestyle. So that's the problem, here is my idea for a solution.
I have recently joined a gym, my goal is 4 days per week, 1 hour or day, light cardio and limited weight training. I will go right after work, which has the added benefit of allowing me to wind down before coming home to a busy house.
My diet will focus at first on cutting sodium and keeping my calorie intake under control. Also eating at regular intervals throughout the day in order to help regulate my metabolism.
My short term goals (6mos) are to eliminate one of my bp meds and lose 20lbs, while improving my overall fitness and stamina levels.
