Starting back after 4 week break

I was on holiday in Europe for 3 weeks during which I did no lifting, though I did do a lot of hill walking. Then a few days after I got back, it was colonoscopy time.(I have ulcerative colitis) Yay!
I found it hard to accurately track my calories while I was away and of course ate next to nothing the day before and day of the colonoscopy. Miraculously I've only lost 2-3 lbs and now know I can easily regain that. Of more concern is that I know I've lost muscle- I can feel and see that I have.
I was doing strong lifts x5 , very slowly due to various things, chronic illness being one. Do I dive back in where I left off ? Or start again ? Am still feeling wiped out from colonoscopy which incidentally showed some inflammation, so I know I need to take it easy. Thanks in advance!

I am older so don't go for fast increases in weight. When I have been on a break for a month or longer I usually do my first session back at the weight before last and see how I feel from there.
Sometimes I can move to my last weight, sometimes I can't.
I just look at it as I am back doing it and will progress when I know the time is right- no rush.
Your strength should pick up reasonably fast, mine does.
