I've gone from 34 inch waist to being able to (just) get into 28 inch and while there is still fat to lose (you can really see it when I sit down) I no longer hate my reflection.
Problem is now that I no longer hate the way I look and feel a lot more confident in myself I am finding it so much easier to ignore my diet.
I really want to shift the last 12 lbs but when I get hungry it is so much easier to justify to myself why a snack is okay.
What would you suggest?

I did that rationalizing last time I lost weight - "I'm thin, I can eat whatever I want!". Of course I didn't stay thin for long. But I wanted to eat whatever I wanted more than I wanted to stay thin. This time I have a diet that I want to stick to regardless of what it makes me weigh, it just happens to also be a diet that makes me stay at a normal, healthy weight.

Oh, but I have never had trouble with hunger. I get cravings, which is a completely different animal, but also just the other side of the coin. Good nutrition, a regular meal schedule and removing temptations as much as possible, calms them down. I often read that hunger is a problem for well fed Westernes, and sometimes it makes me forget that it's not a problem for me - I have to remind myself that I'm being manipulated. I'm fine as long as I eat enough, but not too much.

Why do you really want to shift the last 12 lbs? I think that is where you motivation will come from.
