This isn't always the case, if your skin has been stretched for a long period of time and you are older and you are genetically predisposed you may have loose skin regardless. I have lost 48 lbs averaging 1-2lbs a week and currently have some loose skin, though I am hopeful it will tighten up over time as the skin under my chin was loose for a while but is now non-existent.4

This is not true.6 -
The majority of the loose skin I have is from pregnancy. It was there before I started losing and it hasn't really changed 65+ lbs later.1

Sure. Losing weight slowly does not prevent you from having loose skin. Loose skin is dependent on your age, health, how overweight you were, how long you were overweight, and probably 30 individual ambiguous genetic factors that are outside of your control. Your skin is actually more likely to tighten back up when you are very lean and there is no more subcutaneous fat sagging it down with gravity.
Losing weight slowly might give the illusion that you don't have loose skin, but all you're doing is revealing the loose skin more slowly. It does not prevent it from occurring.10
