Looking to think diet and workout again

Hello all,
I'm looking to change my diet and start a workout. I have a bit more time over the next 2 months to try to "Kickstart" this routine though I do not have a good plan yet on how to do this.
My goals:
-NO more fast food(obvious)
-121lbs to 170lbs
-feel healthy though still able to eat a cheeseburger once things are together
I look forward to your advice and encouragement since I'm usually motivated for a few weeks and lose this focus. Is there such of a thing as an online trainer to guide the nutritional and workout aspects or is this online community truly a great free complete package way to go? I just want to figure this out, thanks!

Just set your Goals on this site. If you have over 75 pounds to lose, set it to "Lose 2 pounds per week." If you have under 25 pounds to lose, set it to, "Lose 1/2 pound per week."
Anything in between 75 and 25, set it to, "Lose 1 pound per week."
Be honest and realistic in your Activity Setting.
When you exercise, enter it in the Exercise tab and eat that amount in addition to your base goals.
Log all your food, even fast food. C'mon, it's gonna happen. Log it.
Study your food page and learn from it. Try to stick with any big changes for a month or more before changing anything else (calorie-wise) so you'll have good data.
Weigh yourself and record it at least once a week. Most of us weigh daily.0
