Do your muscles know the difference between doing, let’s say, 1 pushup per hour for 16 hours versus 16 straight pushups?

Yes, they will be tireder after 16 in a row.5

There is more stimulas to the muscles the more you rep in a set. So the benefits would be next to nothing if you only did one every hour if you could do 16 in one set.

Yes. There will be more fiber recruitment in a set of 16 vs. a set of 1. A set of 1 isn't going to create enough stimulus to trigger an adaptation.3
