I am back for about the 5th time

I am back on myfitness pal and I want to be more discipline and make the lifestyle changes needed. I don't want to be thin just be healthy and maintain a healthy weight. Trying to eat Clean /reduce carb intake and get moving. I have about 25 pounds to lose and not in a hurry just want it to be done naturally and make the changes necessary to maintain.
