Is 45 mins in the gym good enough??

I have been working out in the gym fairly consistently with the last few months and myself and my friends and family have noticed that I have been losing weight and toning up.
I used to always do 15 minutes on the cross trainer and 30 minutes running on the treadmill, and also sometimes and now usually I do some exercises afterwards such as crunches, press ups and using the medicine ball.
However now I am after getting bored of the treadmill so now I do 15 minutes on the cross trainer, 15 minutes of running on the treadmill, 10 minutes on the bike and then finally 5 minutes on the cross trainer plus exercises afterwards.
Just to add during and after every work out I do be positively panting and dripping with sweat because I work very hard and whenever I feel like I want to give up I almost always never do and just keep going.
Sometimes when I am not working out I like to go for a 30 minutes to 45 minute swim in the gym's swimming pool but I have not been doing this consistently but now from now on I plan on doing lighter and more fun exercise in between my work out sessions.
I plan on going to the gym 4 times a week for 45 minutes starting Monday, Wednesday, Friday and Sunday, and in between those gym sessions I plan on doing lighter exercises such as walking or swimming. I also hope to eat a bit healthier too because my biggest downfall is my portion sizes but fortunately I have been cutting them down unbeknownst to myself, I think this may be due to my stomach muscles tightening up from the gym.
However I do try to eat fairly healthily because I do love vegetables and also I like to eat fruit as well. But I think I need a proper diet plan so finally does anyone know of a decent food diet plan that isn't too hardcore but will still help me to steadily tone up and lose weight along with my 45 minutes in the gym 3 to 4 times a week??

I would recommend a program of compound lifts - bench press, deadlifts, squats, chin ups/pull ups, rows, etc...
Learn those moves and incorporate them into a total body routine where you hit legs, arms, back, shoulders three times a week with cardio two times a week ..
you seem to be over doing it on the cardio, IMO...0
