Hello everyone
Inquiring if anyone knows where u can go to find guidance on how to write a 30-60-90 day plan to keep myself on track .... I log in but I think I need to write down more specific detailed goals to keep myself accountable

Look into SMART goals (google it).2

My suggestion would be to write a plan that focuses on the *actions* that you are taking to get the results you want. What those actions should be depends on where you are, and what your goals are. Metrics might be: number of calories consumed (maybe you do that already?), minutes of exercise (or specific exercises like walking) per week (can track this in MFP too), whether you ate within a specified time window if you have one, your performance at various exercises (pushups*, etc.).
The other thing you might have in mind is the milestones you are trying to hit (here are mine; not the best, but an example). I would not focus on those in terms of a plan, though since you can't control the dates when you hit milestones -- but you can control taking the actions that lead to the results you are after.
