A ketogenic diet is one where carbs are restricted to below 20-50g of carbs a day, usually 5-10% of your macros. To replace the carbs one typically eats more fat or leaves a calorie deficit for weight loss.
When you eat Keto, the body is primarily using fat and (body made) ketones for fuel. After a few weeks, the body becomes very good at it and glucose usage by the body drops quite a bit.
Those with insulin resistance health issues tend to benefit the most from a diet very low in carbs.0
