Trying to get back in shape

So I’m a very active person yet when it comes to doing proactive activities such as working out, eating healthy foods keeping my mental positive and staying motivated I fail everytime I try and try but it’s when I don’t see instant results or even results over a period of time I lose motivation fast well I gave it a try last year and I was doing good but then I hit a depressed state and lost my motivation so here I am back at it going to try and give it my all again.. just need help on staying motivated any ideas?
