Depends on the food nutrition labelling in the country that the particular food items you pick from the database relate to.
Most of the world labels food with net carbs.

Remember this is an app with a global user base and most of the food entries are user created.

That would depend on who entered that particular entry and in which country. Some countries use net carbs on their labels, other countries don't.

PS: free version or premium version of the app doesn't matter for this question.

Free or paid version doesn't matter.

What country the label info came from matters.
US is total carbs on labels.
    Other countries vary.

ALWAYS confirm the food you have matches the label of food being eaten.

When you say you manually add, you mean you create the entry? If so, it's whatever the entries you create are, depends on where you are getting the information you are adding.
