So when i load up the bar and do squats, i am getting a really sharp and powerful pain in my left side shoulder blade.
It only happens when i squat and never when doing any other exercise.
Thoughts?

Could be pressing on a nerve
I had a rotator cuff repair in October and my physio guy told me do not do anything with the weights on my shoulders so could be the start of a shoulder issue0 -
Hard to tell with the vague info you have given.
Maybe where the barbell rests. It might be on the spine of your scapula.
Try a different variation, barbell position, grip width, etc...
You can also post a vid on my form check thread and I can give a more detailed response if I have more info.
I'm confident you will find something comfortable if you try.
