Trying to help my mom make an account but it won't let me?

I enter all her info, I get to the last part and click to confirm, but then all that shows up is a message saying " there was a problem with your request"
We've tried this 3x now and always had the same result. Are we doing something wrong or is this a bug on mfp's side?
(mom's an older lady who's not great with computers or english, hence me trying to help - but we're both stumped)
