Gym Excercise Logbook

My PT has come up with a gym schedule for me, 3 days a week, 45min sessions. My gym has these cards where we can keep track of the routine and how much I'm lifting, what ratio(accent to decent) and what rest periods I'm to have. Now the card is small and doesn't have the best UI either. Even my trainer admitted to it being a bit flawed. He recommended keeping a little log book and writing it down. I looked at a few apps but don't like to carry my phone around the gym, prefer to leave it in my car.

So I'm thinking I'd do the logbook idea, but interested to see what designs or templates you guys use to keep track.?

I have a few I like from Google, so I'm take what i like and apply it to an excel spreadsheet and make my own. Hopefully I can show you guys my end results within a day or so.
