Per say, I get a steak and cheese on honey oat bread from Subway. Do I need to add the bread separately or is it already counted with it on the menu?

Probably includes some kind of bread, not sure which. But since the food database is mostly crowd-sourced, it's hard to give you a firm answer. (Some people enter some things more meticulously than others do.)

One option is to go to the chain's own nutrition information. Usually just web searching something like "Subway nutrition" will get you a link from the actual company, high in the search results.

In this case, if you're in the US, maybe this one:

Look at the carbs. The fillings for sandwiches are very low carb. If the macros show significant carbs, then its safe to assume the bread is included.

Looking at macros/micros can show handy clues to food.
Once I figured out how much lemon juice to put in a copycat lemoncake recipe by noting the vitamin C levels in the original and matching to my juice measurement.

That will probably tell you whether some bread was included, which is helpful. But the different Subway bread choices seem to vary (in the 6" size) from 140 to 240 calories (double that for foot long), which will be a significant difference for some people. I don't recall OP's honey oat bread being on their list, so don't know where that one falls.

On most (but not all) subway official calorie count numbers cheese and condiments are not included. Their usual base count is based on white. Depending on franchise don't be shocked if slices are shaved or too generous affecting final counts :) if frequent goto I sometimes invest in sampling and deconstruction. A foot long could be made such as a random half makes it to the counting location a couple of days in a row to establish a base line. If infrequent.... The general count plus cheese and mayo or sauces is generally good enough. Please note that with the newer menus I would expect cheese and sauces included by design to be included in the count... so a bit of a mess 🤷‍♂️

Usually, Subway includes the bread in the sandwich's listed calorie count.
So if you order "steak and cheese on honey oat bread" the calories should already cover the bread. Unless you change the bread type or size, you don't need to add it separately.
It's a good idea to confirm with your local Subway for any menu changes or differences.
