The only thing premium gives you extra for adding food is the barcode scanner and, I believe, the possibility to add timestamps. As for creating foods and searching the food database manually, that's possible in the free version too.
I would recommend searching the food database manually first for an entry with the correct nutritional values before creating foods, the food database is huge already.

Depending on where you live, the barcode scanner may or may not be worth the money. I often get zero results, and then I have to search manually anyway. Or I get a different food entirely, so I have to search manually anyway. Or I get the correct food with incorrect nutritional values - I then use the 'report this food' option to correct and (most importantly) save the food to My Foods.
I'm in Belgium, I think the barcode scanner gives slightly better results in larger (Anglo-Saxon) countries like the US.
