Has any one lost weight at maintenance calorie setting?

Yes
The calorie goals given by any online calculator are just estimates, and so are your inputs to these calculators.
Using varying Activity Levels or different ways to account for purposeful exercise or daily work/school/housework etc. will give different calorie levels.
It's also a certain skill to log food, so lots of mistake are made on that end too.
You have to run an experiment using XXXX numbers and then adjust as you go along.

When using the default myfitnesspal recommendation I found I was going about a 1/2 pound a week, I moved my caloric intake up about 350 calories per day, and adjusted my carb/protein/fat ratios to fit my lower carb intake and I have leveled out.

If you have a 'maintenance' estimate and eat that consistently/correctly and lose weight, it's not maintenance … it's still a deficit. It's just an estimate - so changes can be made based on the results of eating that much.
