Am I Out of Luck?

I calculated my TDEE a little earlier and it was 2060. However, I get up to 1000 calories each day from the dextrose solution I use in peritoneal dialysis. I've been eating under 1000 calories because I have to calculate for the 'invisible' calories I get from dialysis. I haven't been eating back my exercise calories because I fear those are overestimated, but I've been hungry lately. Should I eat back a certain amount?

Just so you know, I'm a 5'4" and 35 years old. My current weight is 210lbs and I'm hoping to eventually get to around 140lbs. I have to lose below the obesity level so I can be eligible for a kidney transplant - so that's under 180lbs. My diary is open, but it may look confusing. Today isn't complete yet, and I have to follow a renal diet with is low phosphorous, low potassium, and higher protein. I also have days I'm simply too sick to eat or to hold food down.

I've talked to both my dialysis dietician and the dietician at the transplant center, and both agree that I should eat around 800 calories. Please note I'm not advocating a VLCD. I don't believe in those, but I have to calculate getting calories from places other than food. Still, I'm starting to really get hungry on some days. I think it's because I'm exercising more. I consider myself sedentary because I stayed hooked to a machine for twelve hours each night (still attached to actually) but I try to get a half-hour to an hour of exercise each day - either walking or biking. Any advice?

    I would seriously talk to your doctor about this. I am pretty sure when you go through dialysis, your metabolic rate severely increases to work to repair your system. I recall seeing a study that discusses this. Last thing you really wan to do is restrict calories when trying to recovery from a medical issue.

    Yeah, but I was told if I didn't show significant weight loss in three months and was able to keep it off, then I would be taken off the list for a kidney transplant permanantly.

    My digestive system doesn't work very well, which I already know, so I can't process foods all that well. I think I need to eat a bit more, but I can't invest in a HRM monitor at the moment so I'm unsure of how many calories to eat back.

    Did your doctor provide a plan or recommendation? If not, maybe he has a suggestion for who you can talk to. I am sorry, I cant add anything constructive. But I know when my friend is prepping her patients for stuff like this, she sends them to a specialist.

    The dietician gave me some papers to recognize hidden phosphorous in foods (my phosphorous was way too high) but that's about it. I showed her my diary I have here on MFP and she said I was doing well. The one at the transplant center only lectures me about portion sizes, which I know so well by now I could lecture her.

    I didn't tell her I was hungry though because this issue has only arisen in the last few days.
