    How can one add a note instead of adding food?

    Do you mean on your food diary? On the web version, scroll down to the bottom of the diary where it says "Today's Food Notes" and click on "Edit note." On the iPhone and iPad apps, there is an option to add or edit a food note, too, but I don't remember where it is.
