Starting Over... Again!

Hello all,
I am a 28 year old, mom of 3 (10, 4, 2) who has lost 50lbs since 2011 (but gained back 5 stubborn pounds). I was doing awesome and felt awesome; until some medical "issue" threw me off my horse again and I am now up 5lbs, that will NOT budge this time and I am finding myself struggling big time with getting that drive back again.
My stats are:
Height: 5'5
Starting Weight: 183lbs
Current Weight: 138lbs
Goal Weight: 128lbs
Ultimate Goal Weight: 120lbs.
I have gone from a size 14, to a size 6-8.
I still have a TON of toning up to do... as my belly is NOT firm at all from 3 babies (wish I were part of the celebrity gene pool) and my body really loves to hold onto fat in my arms, belly & thighs.
I am pear shaped... gotta love the mom hips!
I try my hardest to eat as healthy & clean as possible -- but man, I love food!! And I love to go out to eat... thank goodness my 2 year old is going through the horrible terrible two's and we do not go out much right now, I think that has saved me from another 5-10lb gain... ordering healthy cuisines just doesn't appeal to me. I know, I know... tsk tsk tsk!
I do workout for at least 1-1.5 hrs, 3 times a week or more. My favorites include a Tone & Sculpt class at the YMCA or lifting weights. I want a lean, muscular body... not a skinny skeletal one!
I am looking for other momma's out there that need some support & motivation like I do to lose these dreaded extra pounds!! If you are that momma, please feel free to add me as your friend!!
