Getting heart, blood, and liver healthy

I'm back on the site to lose weight and get in shape. My cholesterol, BP, triglycerides, and blood glucose are all elevated, so I have to lose weight or face serious health problems by my early 30s. I'm 25. Most of my current health issues have just appeared within the last year, while I was in my last year of graduate school, student teaching, and planning a wedding. Quick to develop, slow to get the counts down.

Is anyone else on here to get healthy, and if so, what are some of the things you're doing that work? Vitamins? Supplements? Workout routines? Certain foods that help more than others?

I can't run yet because the shinsplints brought on are too bad, so my cardio is just walking and elliptical interval training at the gym.
